SMS Terms and Conditions of Use

SMS PROGRAM
By opting-in, you consent to receive and authorize Forum Extended Care Services (and its service providers) to send you recurring automated messages related to IT support and emergency announcements related to work conditions at the mobile number you provide during your onboarding. Your consent is not a condition of employment. You may revoke this consent at any time by replying STOP. Message and data rates may apply.
HOW TO CONSENT
By electronically signing the Employee Handbook in Paycom, you expressly consent to receive SMS messages from Forum Extended Care Services for IT support and emergency work condition notifications.
USE OF SMS
Text messaging is strictly used for IT support and notifying employees about work conditions at Forum locations (power outages, road blockages, etc.).
MESSAGE FREQUENCY
Text message is used only for critical IT issues and emergency work conditions and is rarely used.
FOR HELP
If you need assistance, please reply with the keyword HELP or email the IT team at helpdesk@forumpharmacy.com.
CANCELLATION
Reply to a message with keyword STOP, QUIT, END, CANCEL, or UNSUBSCRIBE to no longer receive text messages. After replying or texting the keyword, you will receive one additional text message confirming that your request has been processed.
