With more than 20 years’ experience in pharmacy and having spent the last 15 in long-term care, Johnson Abraham manages Forum’s clinical programs and consultant services. In his role he works directly with facility management to ensure that our services meet facilities’ clinical needs and align with desired outcomes. He joined Forum in 2007 as a Consultant Pharmacist from Enloe Drugs of Northern Illinois / Omnicare, and was promoted to Director of Clinical Services in 2014.

A Board-Certified Geriatric Pharmacist, Johnson received his Doctorate in pharmacy from the University of Illinois at Chicago after Pharmacy and Business Management internships with Walgreens. He is a former President of the Illinois Chapter and member of the American Society of Consultant Pharmacists.

Pam Kramer has been involved in process development and improvement in every company with which she has been associated during her more than 35 years in the pharmaceutical industry. She has worked across the country with agencies, hospitals, healthcare centers, and pharmaceutical/biotechnology manufacturers, creating award-winning strategies and programs in oncology, cardiology, infectious diseases, pharmacy, and more for such noted clients as Abbott Laboratories, Parke-Davis, and Genentech. 

She joined Forum in 2004 as Director of Marketing & Client Relations, working with client facilities to align pharmacy services to customer needs. With a unique background that balances strategic planning and operational process development with a keen understanding of what her customers need, she was named Vice President of Business Operations in 2007 and Executive Vice President in 2011. Responsible for development and implementation of Forum’s overall company strategy, as well as overseeing pharmacy and business operations, she is intimately involved in optimizing infrastructure and services to create exceptional value for customers. 

Pam also serves as the pharmacy’s lead educator, developing programs on compliance, controlled substance regulations, reimbursement, industry trends, process development, and procedural improvement. With particular expertise in electronic health records and reimbursement, Pam has been a speaker at educational meetings, family nights, and conferences such as the American Society of Consultant Pharmacists, Illinois Health Care Association, and Leading Age Illinois. She holds a bachelor’s degree from New York University and is a Licensed Pharmacy Technician.

CC is the driving force behind Quantum Age, a consultancy serving organizations in senior care and aging services. She is among the top strategists in the field, specializing in strategic planning, positioning and branding, and leadership development for the long-term/post-acute care and aging services sectors. CC serves on advisory groups for several organizations, including the Society for the Advancement of Gerontological Environments (SAGE) and Environments for Aging, and is vice chair of the Advancing Excellence in Long Term Care Collaborative (AELTCC). She holds a Master’s degree in the Management of Aging Services from the UMBC School of Aging and a Bachelor’s degree in Communication and Economics from Oberlin College.

With 20 years of experience in business development, with the last 10 years in successful healthcare start-ups, Gina is a thought leader in healthcare business development. She joined Forum in 2019, and is responsible for driving growth and expanding Forum’s market reach. Having successfully navigated business growth for post-acute focused companies like Redilearning/Relias, CareMerge and CipherHealth, Gina’s ability to create unique growth strategies around identified markets has honed her business development expertise. Her success is achieved by working closely with clients to the find the right solutions that ultimately increase their margins and produce higher resident satisfaction.

Gina holds a Bachelor of Science degree in Business and Human Environmental Sciences from the University of Missouri. She is a volunteer youth coach for multiple youth sports associations and is a field volunteer for the Saint Vincent de Paul Society.

Appropriateness of Drug Therapy

Modern medicines have contributed to longer life spans and improved quality of life. However, when not used appropriately, effectively, and safely, medications can have devastating consequences. There are several risk factors that can lead to polypharmacy, including having multiple medical conditions managed by multiple prescribers. Monitoring patients’ active medication lists and deprescribing any unnecessary medications are recommended to reduce pill burden, risks of adverse drug events, and financial hardship. This session will review medications with increased risk of complications in the elderly and presents a process for patient active medication list monitoring and deprescribing as a therapeutic intervention. Pain Management in Older Adults

An estimated 50% of older adults living at home and up to 80% of those living in nursing homes have significant pain that is often persistent. There are many challenges that can make it difficult to treat pain in this age group, most notably the presence of other significant health problems and the difficulty of assessing pain in those with cognitive impairment. This session will review different pain types and discuss the misconceptions and consequences of untreated pain in the elderly. Plants or Pills? The Truth About Dietary Supplements

The use of dietary supplements, such as vitamins, minerals, plant products, and herbal therapies has become a routine part of the American lifestyle. But often, these items are marketed as having benefits that are unsubstantiated; do not carry adequate warnings about when they interfere with lab tests; interfere with prescribed drugs; and may be completely misused by the patient because they are “all-natural” and “must be safe, right?” Health and senior care professionals are relied upon by patients/residents and their families to have accurate information about the uses and effects of these products. Using evidence-based research, we will review the current clinical findings and safety profiles of numerous over-the-counter products used by seniors for various medical and psychological conditions.
