SKOKIE, IL—August 13, 2012 —Forum Extended Care Services, one of the largest independent long-term care and hospice pharmacies has again achieved the Award of Accreditation from the Healthcare Quality Association on Accreditation (HQAA), based in Waterloo, Iowa. This award was presented on July 18, 2012 and is achieved only when a provider demonstrates a high level of quality in all their business operations and services.  Forum earned the three-year accreditation after completing a comprehensive review process that included meeting quality standards and a commitment to quality operational practices and procedures in the areas of financial stability, human resource training, patient care, delivery services, and billing practices. Accreditation by the Healthcare Quality Association on Accreditation assures Medicare and other health payers that the highest levels of quality are practiced within all areas of the company.
